April Simms is a Licensed Clinical Social Worker licensed to practice in Virginia. She graduated in 2005 from Radford University with a Master’s Degree in Social Work. April began her career working at a local Department of Social Services providing services to children, families, individuals with disabilities, and older adults. While working at Social Services, April received intensive training on family dynamics, family and individual trauma, childhood behaviors, and life transitions. April continued her career at a local community services board providing assertive community treatment to adults with severe mental illness (SMI), substance abuse disorders, and co-occurring disorders.

Tonya McFadden, Ph.D. is a doctoral level clinical psychologist licensed to practice psychology in the states of Virginia and West Virginia. She has over 30 years of clinical experience. She has worked in a wide variety of mental health settings including private practice, community mental health, physical rehabilitation, the clinical director of an inpatient psychiatric unit, and inpatient hospital where she performed psychological consultations. In her practice, she has performed psychotherapy and conducted psychological evaluations. She was also a member of the International Honor Society in Psychology.

Chronic Care Management
Bland County Medical Clinic understands that sometimes managing one’s health can require a little extra assistance. Our Chronic Care Management Program (CCM) is designed to help patients with Medicare who have two or more chronic conditions. The program is available to patients at both primary care locations (Orchard Creek Clinic and Bland County Medical Clinic), and is designed to provide support to patients who need to speak with a nurse several times per month in order to manage their health. The CCM program is ideal for the individual who has difficulty managing their medications, coordinating visits with multiple providers, have frequent questions about their medications/health management, or have caregivers who may need assistance. The program is open to individuals with Medicare and at least two chronic health conditions and is intended for patients who need the service to maintain/manage their health and not for patients who are independent without need for extra support.

Bland County Medical Clinic offers in-house x-ray services for our patients at our main clinic in Bastian VA. Our exceptional staff of Radiologic Technologists are available during all clinic hours. We have recently upgraded our X-Ray equipment to the latest digital technology available. Every x-ray taken is sent to a board-certified radiologist group to confirm the findings.

In-House Bone Density
Bland County Medical Clinic offers in-hose bone density scans for our patients at our main clinic in Bastian VA. We have recently upgraded our DEXA scan equipment to the latest Hologic machine and technology available. A bone density scan, also known as a DEXA scan, is a type of low-dose x-ray test that measures calcium and other minerals in your bones. The measurement helps show the strength and thickness of bones. Most people’s bones become thinner as they grow older. A bone density scan is a valuable tool that can confirm or rule out a diagnosis of osteopenia and osteoporosis.
In-House Ultrasounds/Echos
Bland County Medical Clinic offers in-house ultrasounds and echos for our patients at our main clinic in Bastian VA. We have recently upgraded our ultrasound equipment to the latest technology available. BCMC has multiple sonographers on staff, and ultrasounds are available during all clinic hours. BCMC currently does not offer OB ultrasounds. Our Echoes are currently sent to Carillion for interpretation, and all other ultrasounds are sent to a board-certified radiologist group to confirm findings.

Bland County Medical Clinic offers on-site laboratory services through LabCorp as well as Millennium Drug Screenings. Our Lab Technicians are able to obtain a wide variety of laboratory specimens. Lab services are available only to patients of BCMC.
